It is possible to get a similar out of sync if the clock on the phone or desktop is set inaccurately. It could also be a problem if the DSS time is not set correctly. Check the DSS for:
1. Correct UTC time
2. Correct Timezone
3. Accurate local time (i.e. within about 1 minute)
With that established if a problem occurs with a phone, the same time settings should be checked on the phone.
The time is an important part of the AES token checking algorithm.
