Product Defect 350319
This issue is being caused by a defect that occurs when the proxy hostname setting is changed. The CAM database does not properly update the proxy hostname setting.
WORKAROUND 1:
If POC install with localdb then the database cannot be editited, change the proxy hostname setting back to the hostname.
1 - Go to the Admistration Fallback page and login
2 - Go to Settings then click Configure the Cloud Access Manager Proxy
3 - On the Configure Proxy Settings change the name back to the hostname.
WORKAROUND 2:
If it is a Production install, or a POC install with SQL Express (not localdb), you can determine if you are affected by this defect by doing the following:
1 - On the STS server go to SQL Server Management Studio
2- Expand Databases and go to CTData | Tables | FedTrusts\
3 - Look at the row for the proxy application (urn:CloudAccessManager/Proxy) and check that the hostname portion of the responseURL and requestURL fields match. If the defect is present the responseURL field will still show the old hostname in the URL. If so, edit this field to use the new hostname in the URL instead.

Fixed in CAM version 7.0.2.5296 and up
