RESOLUTION 1:
1 - Mouse over the name of the offline proxy server in the CAM Status screen. When you do this a trash can icon should appear on the right.
2 - Click on the trash can to remove the proxy from the list.
PLEASE NOTE: If you use a load balance be sure to remove the proxy from it as well to ensure no traffic get forwarded to the old proxy deleted server.
RESOLUTION 2:
If for some reason resolution 1 is not working, the following steps can be used to remove the proxy host by manually editing your configuration.
It will look something like this:

The Proxy component (ID=5) is the one we're interested in; the other values correspond to the lastTimeOK and runningCfgVersion values for the component in the Database.
