You may have subjected your Password Manager instance to an internal or external penetration/security test and noticed a flag of a potential Captcha or Recaptcha bypass. For example:
Incorrect product’s configuration. Password Manager is configured to display a security image only during the initial search, allowing all subsequent searches to proceed without that challenge.
Please perform the configuration adjustment below and resubmit your PM instance for testing, this should address the flag that you've have received:
1) Launch the PMAdmin site and navigate to: General Settings > Search and Logon Options;
2) Scroll down to the section: 'Security Settings';
3) Make sure the 'Show a security image every time the search is performed' checkbox is selected;
4) Click 'Save'.
