The OPR QR code being cut off at the top making it unusable, the sizing is not correct -
Cause
The issue has been identified as a Change Request TF00239501 and has been raised with our Product team for consideration of inclusion within a future release of Password Manager.

Resolution
WORKAROUND:
Note : screen resolution should be 100% we have mention the admin guide (Refer admin guide page number : 34 under the "Important" section).
If issue still persist then workaround, use the OPR 32 bit challenge code Option.
STATUS:
Waiting for fix in a future release of Password Manager
