RESOLUTION 1
Ensure that the necessary permissions are set for the Password Manager Service Account
RESOLUTION 2
1. Create the following four registry keys on the Password Manager host(s):

2. Reboot the Password Manager hosts

In certain environments, Password Manager may fail to obtain the Site information from Active Directory. This function is referred to as SiteCost in which Password Manager determines the best Site to use to connect to Domain Controllers.
To disable the SiteCost feature in Password Manager, follow these steps:
1. Launch Regedit on the Password Manager host(s)
2. Navigate to HKEY_LOCAL_MACHINE\SOFTWARE\One Identity\Password Manager
3. Right-click and create a new String with a value of: SiteCost
4. Set the value to: False
5. Restart the Password Manager Service after the change
