If a user is attempting to unlock their account via the ‘Unlock My Account’ workflow and they enter the wrong answers in their Q&A profile 3 times then the workflow has failed.
The user should receive an email for each failed attempt and the administrator should receive a single email. The email is not being sent to the administrator.
