Authentication fails on Redhat 6, proftp fails, sshd fails
Deprecated pam-stack module configured in /etc/pam.d/<filename>. In RHEL-5 the use of pam_stack was replaced by include and the pam_stack module was deprecated. It is no longer present in RHEL-6.
The bad configuration in /etc/pam.d/proftpd looked like this:

auth sufficient pam_vas3.so create_homedir debug trace
auth requisite pam_vas3.so echo_return debug trace
auth required pam_listfile.so item=user sense=deny file=/etc/ftpusers onerr=succeed
auth required pam_stack.so service=system-auth
auth required pam_shells.so
Edit the /etc/pam.d/<filename> in the above case it was the /etc/pam.d/proftpd file but it could be missconfigured in other pam files as well:
#cat /etc/pam.d/proftpd

auth required pam_listfile.so item=user sense=deny file=/etc/ftpusers onerr=succeed
auth include system-auth
auth required pam_shells.so
account include system-auth
session include system-auth
