This issue was being caused by 3rd party Anti-virus application.
RESOLUTION 1:
Symantec:
When we stop the antivirus program /etc/init.d/symcfgs stop, the issue went away. When we start them ( one command to stop, but 3 to start them ), the issue returns after running:
The issue has been resolved by installing the 2 anti-virus packages as recommended by Symantec.

RESOLUTION 2:
Add exception for the QAS database file which are the QAS cache. They are located in /var/opt/quest directory and have a vdb extension.
