On 6.10 webclient unable to click "Submit Request" on a "User Supplied" access request

When using a "User Supplied" access request, and requesting the access from the web client at the point submitting the request, the "Submit Request" button is greyed out and cannot be clicked.

In Access Requests | My Requests | Settings cog, select "Use Previous Version"
