This error prevented users from logging in with an AD account.
Upgraded QAS and there was two accounts in the cache for every user.
WORKAROUND:
/opt/quest/bin/vastool flush
If the problem persists a Support Case would be recommended.
issue was seen when upgrading from QAS 4.0.1.22 TO QAS 4.0.1.52
