Verify that both the "Option" and "Key" fields have the correct information required for the specific patch being applied. These details are available In the patch description. 

If the field is supposed to be blank, ensure there are no characters entered (eg, spaces).  If it should be "/genkey", ensure it is not "\genkey".  Also verify the "Option" and "Key" entered have no leading or trailing spaces. These fields are case sensitive.
