Unable to reset passwords via the web designer password reset portal (4377297)

When using the web designer password reset portal and attempting to reset a users password the following error is displayed:

an exception has occurred while running the form method
F1_ct100_ControlRef15_ControlRef15_Main_Main_Button3_Method.
Object reference not set to an instance of an object

The angular password reset portal is not affected by this.

Password Reset Portal has missing configurations

Please ensure that the following are set:

The following configuration parameter needs to be populated

QER\Person\PasswordResetAuthenticator\ApplicationToken

The following needs to be added to the web.config of the Password Reset Portal:
